Referring someone
For support coordinators, plan managers, local area coordinators, discharge planners and aged care assessors. You are working under time pressure, so this section is the least lyrical thing on the site.
Ring and ask. It moves week to week and by area, and we would rather tell you the truth on the phone than publish a number that is wrong by Thursday.

Brisbane, Sydney and Melbourne plus surrounding regions. If your client is on the edge of a coverage area, ask before you send the referral.
Discharge referrals
Send them as early in the planning as you can. A day or two of notice is the difference between a smooth first week and a scramble.

Funding type and management: NDIS self, plan or agency managed, or Support at Home
What support is needed, how many hours, and which days if that is fixed
Anything that changes who we can roster: language, gender preference, pets, stairs, behaviour support needs
Risks and clinical needs we should know about, and who is handling the clinical side
Your name and direct number, so we are not chasing a shared inbox

Three things worth knowing before you call
Interpreters and access
If English is not your first language, tell us and we will arrange an interpreter through the Translating and Interpreting Service rather than relying on a family member to translate. TIS National can be reached on 131 450. If you are deaf or have difficulty hearing or speaking, the National Relay Service can connect the call. Ask us for information in a different format and we will sort it out.
Grove is support work, not a clinical service. Occupational therapy, physiotherapy, speech pathology, psychology and dietetics all sit with Constellation Therapies inside our own group, and nursing sits with your health team. Ring us anyway and we will make the introduction rather than send you back to a search engine.
Tell us first if you can. Ring the office or email us, ask for it to be treated as a complaint, and we will put it in writing and tell you what we are doing about it. You can also go straight to an external body without telling us at all, and you can do that at any point. For disability support that is the NDIS Quality and Safeguards Commission, and for aged care it is the Aged Care Quality and Safety Commission. Both take complaints from anyone, including anonymously, and both list their current phone numbers and online forms on their own sites.
In business hours, the same day wherever we can. If you send an enquiry over a weekend it will be picked up on Monday. If it is urgent, ring rather than email, because a phone call jumps the queue in a way an email cannot.
What will the first conversation actually be like?
Fifteen or twenty minutes, mostly us listening. What a hard day looks like, what you have already tried, who else is involved, and what you want to be different. No sales pitch and no forms to fill in while we talk.
How long until support actually starts?
Once we know what you need and how it is funded, the limiting factor is matching a worker rather than paperwork. A straightforward start is usually a week or two, and we will tell you at the first call if yours will take longer.
What if Grove is not the right provider for me?
Then we will say so, and point you somewhere better if we can. A referral we cannot staff properly is worse for you than an honest no, and it is worse for us than an empty roster.
